RCGP Wales Advance Care Planning Training Resource

Ensuring good end of life care should be a priority for any health and social care professional who cares for individuals in the latter stages of their lives. To ensure that the care we deliver is person centred, we need to involve the individual in planning their care – supporting individuals to complete an Advance Care Plan is one way of doing this.  RCGP Wales and its partners hope that this training resource gives health and social care professionals the confidence and skills to deliver person centred care through Advance Care Planning.

 RCGP Wales, in conjunction with the GMC, Marie Curie, RCN, Macmillan and the Stroke Association have developed a free interactive Advance Care Planning Training Resource which is aimed at healthcare professionals across the UK from a wide variety of different backgrounds and experience, including people working in primary and secondary care, and in residential care settings.
