Sometimes your health and social care practitioners can keep a paper or electronic copy or summary of your Advance Care Plan documents. This is very helpful as it means they can share with others involved in your care. You could also:
- Keep them in an easily accessible place in your home.
- If you have a district nurse keep them in their notes in your home.
- Think about using and advising in a note on your MedicAlert bracelet or the Lions ‘message in a bottle’ where your plans are.
- You can also store a copy of the Advance Care Plan passport your wallet or purse.
It is helpful to keep your original Advance Care Plan documents in your own home letting your family / loved ones know where to find it.
