There should be no charge for an Advance Statement or an Advance Decision to Refuse Treatment as the documents are freely available unless you ask e.g. for specialist advice to complete them. Some solicitors may charge a fee for their advice and there are some websites that charge a fee for the use of their documents and expertise.
Registering a Lasting Power of Attorney (LPA) for Health and Care Decisions with the Office of Public Guardian will cost a (means tested) fee. A solicitor will also charge you a fee to help you complete and register the LPA.
