Advance Care Plans & young people

For those interested in advance care planning for young people there is documentation available for Advance Care Plans used across Wales in the form of ‘paediatric advance care plan’ (PAC-plan for short). This is in use for children and young people.

Pac Plan

The documents have been developed by experts on an all Wales palliative care network.

They include a helpful leaflet for parents with children and young people with a life limiting illness. This is available here.
